Proof of Residency:
This shows your Peace River STREET address. A box number is not Proof of Residency.
- ID with your street address
- Lease or a letter from your landlord
- A government letter with your name and street address
- A utility bill. Utility bills will show the service address even if mailed to a box number.
Proof of Eligibility:
Upload the documents required for the category you are applying for:
AISH Recipients: A recent AISH statement or a letter from your AISH worker.
Image- Low Income Family or Individual: Notice of Assessment or Income Tax Summary for the current year, for you and your partner if applicable. Your document must show your name, the tax year and Line 15000 with the income amount. If you are taking a screenshot ensure all information is visible. You can get your Notice of Assessment from CRA My Account or by calling 1-800-959-8281.
- NLC Students: Letter from Northern Lakes College showing your enrollment dates.
- Seniors: Proof of age (like ID or birth certificate) and Notice of Assessment or Income Tax Summary for the current year for you and your spouse or partner, if applicable. You must submit your spouse's income even if you are living separately because one of you is living in a supported living facility.
