Have Your Documents Ready

Please read the requirements below for the category you are applying for.  You must attach the required documents or your application cannot be processed. 

Do no upload documents such as pay stubs, bank statements, etc. 

You must submit the appropriate documents listed below.  Please contact Community Services at (780)624-1000 if you have questions or your income has changed since filing your taxes.

 

  • Complete the form to apply for the first time or to renew for another year.
  • It will take up to 5 business days to process your application.  We will contact you when it has been reviewed.
  • Using Your Phone: You can take pictures of your documents or upload photos from your gallery or files. Ensure the pictures are clear and readable.

DOCUMENTS REQUIRED

1 - Proof of Residency: 

This shows your Peace River STREET address. A box number is not Proof of Residency.

  • ID with your street address
  • Lease, dated within the last year.  If your lease is older than one year you will also need to provide rent receipts from the last month.
  • A utility bill. Utility bills will show the service address even if mailed to a box number.
  • A government letter with your name and street address
  • Letter from your landlord confirming your residence.  Your landlord must include their name, address and contact information.  The landlord must be the property owner or manager, a roommate or subletter cannot write a letter for you.

2 - Proof of Eligibility:

Upload the documents required for the category you are applying for.

  • Low Income Family or Individual: Notice of Assessment or Income Tax Summary for the current year, for you and your partner if applicable.  Your document must show your name, the tax year and Line 15000 with the income amount.  If you are taking a screenshot ensure all information is visible. You can get your Notice of Assessment from CRA My Account or by calling 1-800-959-8281.
  • NLC Students: Letter from Northern Lakes College showing your enrollment dates.
  • Seniors: Proof of age (usually ID) and Notice of Assessment or Income Tax Summary for the current year for you and your spouse or partner, if applicable.  You must submit your spouse's income even if you are living separately because one of you is living in a supported living facility.
  • AISH Recipients:  A recent AISH statement or a letter from your AISH worker.
